School Nurse

Students must be healthy to be educated and educated to be healthy!
 
Below you will find important issues and helpful links to ensure your child's health and safety while they are in school.  Communication with the school nurse is essential in addressing your child's personal health needs so they can come to school and stay in school.  Please inform the school nurse of any allergies, surgeries, accidents, injuries, or significant health history that your child might have.  Please update the school nurse of any new health concerns during the summer months or which may occur during the school year.
 
Every student entering Christ the King HS is required to have a complete physical examination including an eye exam.  This must be performed by a licensed practitioner, signed and stamped by his/her office.  This exam must be completed and dated between June 1st and August 1, 2018.  All documentation including vaccine records must be submitted to the school nurse by August 10, 2018.  No student will be permitted to attend school in September unless all requirements have been satisfied.
 
All athletes are required to have an updated physical every year.  Do not wait until the season starts to get the required physical.  You will miss out on tryouts, practices and games.  If you are not sure if your physical is up to date, please contact the school nurse.
 
Incoming freshman must submit a notarized emergency card to the school by August 1, 2018.  An updated emergency card must be submitted if any contact information is changed at anytime for all students.  This is very important in case an emergency situation arises with a student and we need to contact a parent/guardian.
 
Meningitis Requirements for Seniors
All seniors must have the Meningitis Vaccine before the 2018/2019 school year starts.  A letter was sent home at the end of the school year.  If your child has not yet received this vaccine on or after their 16th birthday, they must see a physician to administer this vaccine.  Please send in documentation to the school nurse before the fist day of senior year or your child will be excluded from attending school until this requirement had been met.  If you are not sure if your child has the proper documentation on file, please contact the school nurse.
 
All students requiring medication to be taken in school, prescription or over the counter must have a Medication Permission Request Form completed and signed by the prescribing practitioner and a parent/guardian.
